Add date fields (month/day/year dropdowns and clickable calendar), perhaps under "Meeting" field, for "Arrival Date" and "Departure Date" on form.

Yes, these should be required fields.
Fields added (on DEV).
Ready for testing on QA.
New fields currently implemented in admin UI, as there are unanswered questions about programmatic modification of a webform:
field names: arrival_date, departure_date
field type: Date
website timezone
fields mandatory
start date: -1 years
end date: +2 years
enable popup calendar
This looks good on QA.
Verified on production.
