CDR Tickets

I have been able to add a new CDR user  (Carol Lujan) to QA without problems but I am unable to add her to PROD. There was some confusion with her username (lujanca), the correct one. But what I was provided initiially was (lujanc). I think in the process of trying to correct that, I may have duplicated her account. I inactivated one of them and yet could not fix the remaining one. I deleted the remaining one and now when I add her account again, I am unable to see it in the list of users. Also in the process, I may have deleted the account of Carolina Spencer (spencerct). Could you please restore that account?
