The GP Mailers still has the pre NVCG header information including the previous NCI logo and design. Could you please change it to reflect the current Cancer.gov design?

I have reviewed this on DEV. The new header is showing as expected but there is no title for the mailer. It should have the "NCI Cancer Genetics Services Directory" heading at the top of the page.

This is note for ~MBeckwit The expiry date on the mailer says OMB No. 0925-0639 Expiry Date 08-31-2014. Do we need to update the expiry date?
