The text change will have to be done in the Word document and would then require to convert the document to PDF. This would mean that I'll have to do all of the changes to the PDF document again.
As for the checked boxes, I agree, it would be better to leave them
unchecked but these are Yes/No radio-buttons and in Acrobat one of them
has to be checked.
I've tried to replace the radio-buttons by regular check-boxes but that
created lots of 508 problems which I wasn't able to solve quickly.
Unless you feel these issues to be important enough to start over and change before publishing the document on Cancer.gov I would leave the modification for version 2 at a lower priority task.

If I understand it correctly we are not allowed to request the licensees to fill out the Test License Agreement and we don't need this form anymore.
I'm marking this as RESOLVED/WONTFIX for now but it can probably be closed if Lakshmi agrees.
The process to become a distribution partner has been modified and we're not using an application form anymore.
