The hotel request form and reimbursement request form are currently missing on QA. We regularly use the QA site to train new Board members (including two trainings today) so I'm giving this a higher priority even though it's just on QA. It looks like these forms are available on the other tiers, so we'll use one of those in the meantime. Thanks!
It turned out that all I needed to do was run
drush updatedb. However, I'd like to keep this ticket open for a bit, because I realized as I was doing my sleuthing that the recent security updates for third-party module vulnerabilities actually changed some code that in our version control repository, even though we don't own it, so I need to update the Subversion repository. I'll leave the ticket open as a reminder to do that, and close it when it's taken care of.
I have completed the work on updating the Subversion repository, tagging the results as ebms-3.1.1, so you can close the ticket when you have confirmed that webforms are working on QA.
I've verified that the hotel and reimbursement forms are back on QA.
