I've pasted an e-mail from Victoria below, which summarizes the problem:

While QCing the reformatted NHL summary on pub preview, I noticed that the SummaryFragmentRef links in ordered and itemized lists are not working or even showing as links. The links in the table in the Treatment Option Overview section aren’t showing either. I ran the bold/underline report for the summary and the links show there. I also ran pub preview for SCLC and the links show there as well. I showed Volker and he asked me to send you an e-mail so you could put in an issue for this. The summaries and versions we looked at are:
NHL (721833) V18 – links not showing in pub preview
SCLC (62945) V161 – links do show in pub preview

I also tried another summary (Cancer Genetics Overview - 517309) and the links displayed in PP. So, it may have something to do with the NHL reformat being a new summary document. ❓

That's exactly right!
When I talked to Victoria we talked about this being a reformatted
summary but I didn't realize until you said it, Robin, that this is a
"new" reformatted summary.
Since the vendor filter strips links to summaries that are not
publishable - in order to prevent broken links - and the PP report uses
the vendor output these links will only be displayed
after the first publishable version has been
created.
